Thank you for taking the time to complete our new patient forms in advance of your visit. There are a few steps that many patients miss, so please make sure that the following occur or we may not receive your forms.
- After you complete all the required fields and submit them, you should receive an email asking you to verify your identity. This may take a few minutes before arriving.
- After you verify your identity through the link you received, you will be emailed a copy of your responses. This may also take a few minutes before arriving.
If either of these steps did not occur, please do not close this screen and try to submit your form again.

Financial Responsibility
The Center for Wound Care & Hyperbaric Medicine understands the importance of providing excellent care as well as communicating effectively with patients about their insurance and financial obligations, our goal is to keep an open discussion with our patients about their benefits.
We accept most major insurance including Medicaid and Medicare. Please visit the insurance information page to get a full list of accepted insurance. Our Patient Benefits Representative will call your insurance plan and obtain your coverage and benefits prior to your procedure.
In the event that there are non-covered services, The Center for Wound Care & Hyperbaric Medicine provides payment plans after your insurance paid its portion. We also accept all major credit cards.
